Completion of the annual registration process is required both for new and for returning students. For returning students, this process verifies emergency contact information, health information, and permissions for the health office. Completing this process helps us keep your child safe.
2025-2026 Returning Student Registration Information
We will be conducting online registration again for returning students. Parents completing annual registration online will not need to complete paper forms.

New Student & Kindergarten Registration Instructions
New student registration is completed using paper forms. The parents of all new students must provide the following information:
REQUIRED DOCUMENTATION: All required documentation must be submitted before your child/children begin school.
- Your child's ORIGINAL Birth Certificate with a seal. We will make a copy for our records.
- Proof of Residency-(i.e. Signed Purchase & Sales Agreement to be updated with a Settlement Sheet upon closing, a recent property tax bill, or if renting, a rental agreement). If you are living with a related or non-related resident of Chester, please provide a Notarized Statement with the owner's recent property tax bill.
- Your child's up-to-date immunization records, and proof of a recent physical within a year of entering Chester Academy. This information must be reviewed, and approved by our school nurse, before your child/children may enter school. Please call the health office if you have any questions or concerns at x135.
In addition parents/guardians will need to complete the following forms:
- Data, Contact, and Dismissal Form
- Health History Form
- Multiple Response Form
These forms must be returned to the main office prior to the start of the school year.

Preschool Lottery Registration Application
Our preschool program provides a nurturing and engaging environment for young learners to begin their educational journey. Enrollment is determined through a lottery system to ensure a fair and equal opportunity for all interested families.
Enrollment Process
